Health and Your Community Survey

The S.A.V.E.T.H.E.M Health and Your Community Survey will help us understand and identify issues that reside in various communities and neighborhoods. This survey will also serve as just a quick glimpse of how residents view their communities. We are very thankful for all and information you share with us.

In order to be eligible for the contest drawings that will happy on Fridays until September 9th, you must answer all 5 questions thoughtfully.

The drawings will be August 26, September 2, and September 9. Three lucky winners will be selected to win an Amazon $50 gift card. We will notify the winners through email.
